Consent for Emergency Medical Treatment & Liability Release

I do hereby give authority to the Brooklyn Acting Lab staff to obtain necessary emergency medical treatment for my child with the understanding that the family will be notified as soon as possible.

I have enrolled my child in a program of physical activity including dance. I understand the inherent risks including personal injury that may result in my child’s participation in this activity. I assume the risk and hereby affirm that my child is in good physical condition and does not suffer from any physical disability that would prevent him/her from participation in this program. I hereby agree, that I, for myself, my children adopted or otherwise, my heirs or executors, waive and release any and all rights and claims for damage that I may have at any times against Kristen Lynch, Brooklyn Acting Lab, or the agents or representatives for injury or damage in connection with my child’s entry in activities sponsored by Brooklyn Acting Lab. I have read and fully understood the above.

  • Media Release

    I hereby consent to the taking of photographs, movies or videotapes of the Child named above by Brooklyn Acting Lab and its staff and agents acting on its behalf with its permission. I also grant to Brooklyn Acting Lab the right to edit, use, and reuse said products for non-profit purposes including use in print, on the Internet, and all other forms of media.

    I understand that work or photographs will not be captioned with my child’s full name, and that no other personally identifiable information will be released or posted.

    Drop-Off and Pick-Up:

    The PLAY in a Day program begins at 9AM each morning. Please escort your child into the space and sign your child in each morning upon arrival. For safety reasons, please do not leave children to enter the space on their own.

    You are free to get your child situated and spend time with them in the space until we begin the first group activity. Our first activity is typically a circle- based warm-up game and this will begin between 9:15 and 9:25 AM. If you need to drop-off and leave immediately that is fine too. We will have activities set up for the children every morning before the warm-ups begin.

    If you arrive late (after 9:15am) please just drop off your child.

    Pick-up is at 4:30PM. Your child will only be released to Parent 1, Parent 2, Emergency Contact, and additional authorized adult. If there is any change (ie. playdate) we must receive it in writing. We appreciate your understanding on this issue. If you are running more than 15 minutes late please call the BAL cell at 718-238-8383. After 5pm there will be an additional fee of $5/15 minutes if your child is not enrolled in aftercare.

    If you plan on picking up your child earlier than 4:30PM please let us know in advance if possible so that we are sure to have your child ready.

    $10/day

    Aftercare:
    Monday through Thursday (There is no aftercare on FRIDAYS)
    5-6PM
    $10/day

    Children must be registered in advance for early drop-off and Aftercare.

    Dress

    Please send your child in comfortable clothes and shoes. They will be moving around a lot. Sneakers and supportive sandals are best. No flip-flops please. Since we spend time outdoors (weather permitting), they should bring a jacket. Please label all of their personal belongings (backpack, lunchbox, clothes, etc).

    Lunch/Snack

    Your child should come each day with a nut-free lunch, and two healthy (nut-free) snacks. We eat our first snack between 10:00 and 10:30, lunch at 12:00, and a second snack at 2:00. Please also send a water thermos/bottle which we will refill water bottles throughout the day. We do not have a refrigerator to keep lunches cold and we are unable to heat up food. Also, please do not pack any glass containers in your child’s lunch.

    Health/Safety

    We are committed to providing a healthy and safe environment for your children. In order to do so we ask that if your child is ill you keep them at home until they have fully recovered. If your child gets sick over the course of the day we will contact you to pick him/her up.

    Each morning we will set up the safety groundwork for the children so they understand how we expect them to behave indoors and outdoors. There will always be at least two adults when we bring the children out to a grassy area in Prospect Park.

    In the event of an emergency the child will be brought to the nearest emergency room, which is Methodist Hospital. A parent and/or emergency contact will be notified immediately.

    Communication

    Communication is key. We want to insure that the program is a positive experience for your child. If there are any issues before, during, or after the program please let us know. We are always available to discuss any matters of concern. We love to hear your positive feedback AND constructive criticism. You are our community and open communication with you is essential to our existence and growth. We value what you have to share!

    We know from experience that this age grocan be a time of enormous growth for children. They can take leaps and bounds. We hope that you will discuss with us any challenges your child might have so we can playfully and gently work with them. We find that through creative play, some of the shyest children take on leading roles and some children who typically take on leadership roles grow as listeners and collaborators. Children are also exploring new things and discovering new interests: some children discover they love puppetry or writing stories. We will keep you updated as we witness new discoveries children make over the week.

    If you would like to speak to any of the teaching artists, we ask that you do so very briefly at drop-off or send us an email and we can set up a time after 5PM. Teaching artists will NOT be able to speak to parents/caregivers at pick-up. This is a busy time and they are focused on making sure each of the children leaves the space safely.

    Sharings

    Each day, we will have a sharing at 4:30 PM. We would love it if you could join us at that time, however we do understand that scheduling is sometimes challenging. We are present for children as their “audience” and take lots of photos that we can then share with you. If you wanted to come at another time to observe the class then let us know in advance and we can arrange a time.
